About PMS Hospice

Hospice care makes every day matter. Hospice services in Santa Fe County help patients make the most of every day of their lives. Managed by Presbyterian Medical Services, a nonprofit organization, The Hospice Center provides personalized, expert care for all of our patients.

Hospice is a philosophy that focuses on caring, rather than curing, during the final stages of life. We help patients maintain dignity and improve their quality of life when faced with a terminal diagnosis.

Hospice allows families and loved ones to share their last weeks or months in a home or home-like setting, rather than a hospital. This helps provide comfort to both patients and family members alike.

Our care team is the heart of hospice. A team of interdisciplinary caregivers works together to provide the most complete care possible. That team includes a Medical Director/physician, nurses, MSW-social workers, home health aides, a physical therapists, pastoral and grief support counselors, and volunteers. Palliative care physicians and nurse practitioners make home visits to our patients.

This team creates a circle of care, covering the physical, emotional and spiritual needs of our patients and their families. We’re available 24 hours a day, 365 days a year.
