Time is running out to participate in CHRISTUS Health’s Community Health Needs Assessment Survey! The survey closes on December 17th, and we need your input to better understand and address the health challenges in our community.

This is your last chance to share your voice on key issues like access to care, chronic disease management, and mental health. Your feedback will guide us as we work to eliminate health disparities and improve services for our patients, and community members, and Associates (employees).
