We invite you to participate in CHRISTUS Health’s Community Health Needs Assessment Survey. Your input is essential to understanding the health and social challenges our communities face, and your feedback will directly shape how we address them.

This survey is a critical part of our mission to promote healing and eliminate health disparities. We’ll ask about topics like access to care, chronic disease management, and mental health services. Your voice will help us pinpoint where improvements are needed most, ensuring that we provide better support to patients, community members, and Associates (employees).
